The intake form takes about ten minutes. It captures your history, symptoms, and goals so your first appointment can begin where it should: with what brought you here.
What it costs
Intake $350. Medication management $175. Ketamine inquire-within. Some insurance, HSA, FSA, limited sliding-scale.
How long it takes
The first appointment runs about an hour. Follow-ups are shorter, usually monthly.
